Personal Medical Information Sheet

Who speaks for you when you are unconscious or otherwise unable to communicate with a paramedic?  Everyone at any age should complete this form. Store it on the refrigerator and place a second copy over the visor in your car. Making it available to paramedics entering your home could considerably reduce the time they spend acquiring medical information about the patient prior to treatment. This could even be a life saving document if the patient is unconscious and no one is familiar with their medical history or medications.
